Case Manager Toolkit
This toolkit is designed for community-based practitioners working in a case management setting with couples. It will help guide the worker to web-based resources and a wealth of information, products and materials that further support case managers in their work with couples in a one-on-one setting.
Ultimately, our goal is to provide resources that:
Familiarize practitioners with the needs of couples around strengthening their relationship.
Provide activities to be used in a one-on-one setting with couples to discuss common relationship challenges.
Build skills among couples to manage their relationship.

This guide offers a list of low-cost or free curricula to support efforts to strengthen relationships for singles, couples, parents or families. It will help you find Web-based and other resources including materials developed with federal funding that are available for public distribution. It is the second in a series of guides designed by Administration for Children and Families' (ACF) Healthy Marriage Initiative staff to support the implementation of valuable programs under the Healthy Marriage and Responsible Fatherhood Initiatives.
Resources are available to assist healthy marriage programs in delivering and enhancing services to Hispanic couples and families. This document is not an exhaustive list of available resources but provides general references that may be of interest to practitioners. Programs are encouraged to explore the websites and contact healthy marriage experts for more information.
25-year career in Human Services which began as a case manager has provided him with a wealth of experience and knowledge in working with families. His resume includes project management and administration of programs such as Head Start, Healthy Families and Family Expectations. Over the last 7 years, he has played a major role as an administrator of several national marriage education demonstration projects. His experience in this field has led him to become nationally-recognized for his knowledge and skills in these areas. He has served as a lead consultant to multiple national demonstration sites, providing technical assistance and serving as a specialist in the field at multiple conferences and seminars related to the implementation and management of relationship and family service projects. He has also received training in a number of marriage education curriculums, serving as both a facilitator and advisor to other workshop leaders. Jay has an Associate of Technical Arts Degree in Human Services Administration and received his Bachelors of Arts in Human Services Administration from Evergreen State College in Olympia, WA.
